Right-click on the desired link. If you are using Internet Explorer, choose Save Target As. Save the file in your template directory for Microsoft Word (i.e., c:\winword\template or c:\msoffice\template). If you are using Netscape, choose Save This Link As and follow the same procedure.

When you are ready to fill out the form, create a new document in Microsoft Word by selecting File, New. When the template box pops up, select either modlform or affidavt. You will then have a new document (form) that is ready to be filled out by tabbing from field to field. (The form can be used over and over by following the steps in this paragraph.)

NADOA Model Form Division

Affidavit of Death and Heirship Form
Download a copy of the affidavit. Complete and mail to us along with the death certificate and other pertinent estate information so we may distribute the decedent's interest to the appropriate heirs. (Must have Microsoft Word)
